Nondisclosure Form
I, a student/participant in the Human Study educational program, understand that all data I will have access to and come in possession of during the entire duration of the educational program is to be used only for the purposes of my participation as a student in this educational program. I understand that I am not allowed to share with third parties (persons outside of Human Study) any data that I come in possession of during and in relation to my participation in the educational program, without the written consent of Human Study. I understand that to share the data I come into possession of during the educational program, I must seek written consent from Human Study.
This data includes, but is not limited to:
- Educational content: written and/or video and/or audio and/or digital content,
- Photos and videos made during practical workshops, exams, and all other activities organized by Human Study, and those that are part of case presentations or assignments made by my peers or lecturers and shared with me on the educational platform or other means of internal communication;
- Personal information about my fellow students (including names, employers, information about the progress of education, etc.);
- Any non-educational content (contracts, etc.) that I come in possession of due to my privileged access as a student.
I understand that it is my responsibility to handle images of patients, students, or any other person who appears in the images in accordance with consent signed by the patient and relevant laws on sharing personal information and images. I understand that these people have given their consent to Human Study only and that I do not have any rights to their use or sharing for any other purpose outside of this educational program.
